To place a client on the waiting list the following is needed:

• Client screening completed
• Drug & Alcohol PPC-2R assessment with placement criteria, to obtain a copy, click here.
• Release of information for person(s) who may be checking the status of potential client on the waiting list.

Prior to a bed date for entry the following is needed:

• Copy of current OHP card (Oregon Residents)
• Back OHP premiums paid and proof of payment (Oregon Residents)
• Resources to pay monthly OHP premiums (Oregon Residents)
• List of medication currently taking
• Resources to pay for medication while in treatment
• Pictured ID
• Copy of social security card
• Current TB test
• Current Medical History and Physical
• Tribal ID (if applicable)
