REFERRALS
Youth can be referred to Wemble House through a number of agencies
which include, but are not limited to:

Wemble
House offers Level III Residential Treatment only. Therefore, youth
entering our program must meet the ASAM Level III placement criteria.

INTAKE PROCESS
Once a referral has been made, the Intake Process begins. Youth will
not be placed on our waiting list until Steps 1-2 have been completed.
A confirmed "bed date" will not be issued until all steps have been
completed. The referring agency or person(s) are responsible for
ensuring that all documents are complete.
Step One - Intake Packet
The Wemble House intake packet requires necessary information
such as releases, contact information, medical
history, travel agreement, etc. This packet must
be completed in its entirety and returned to our
Intake Coordinator. To obtain a copy of the Intake

Step Two - ASAM Assessment
Youth
entering Wemble House must meet the ASAM Level III placement criteria.
The assessment is included in the intake packet from pages 25-44. If
you have questions regarding this section, please contact our Intake
Coordinator, Lynn Cunningham.
Step Three - Picture ID
Picture
ID is a requirement. Youth are required to show ID for a number of
reasons, including but not limited to, GED testing, travel purposes,
check cashing, etc.
Acceptable forms of identification include:
1.
Tribal ID (must be current w/picture) 2. State Issued ID (see local DMV
for requirements) 3. State Issued Drivers License 4. Recent School ID
Step Four - Medical History & Physical
Medical
History documents are included in the Intake Packet. However, it is the
referring party's responsibility to ensure the youth has documentation
of a current physical (w/in last year)
Step Five - TB Test or Chest X-Ray
TB
Test/Chest X-Ray is required by the State of Oregon. Documentation of
such tests can not exceed one year from the date of testing.
